Patient and Family Bill of Responsibilities

Make the right one with Wellness Doctor and embark on a path to total health.

Being a valued patient of Wellness Doctor Home Health Care, you and your family have the following Responsibilities:

  1. Show consideration for others and treat the clinical and non-clinical staff with respect.
  2. Respect the privacy and comfort requests expressed by the patient at home.
  3. Provide complete and accurate information about present complaints, past illness, previous hospitalization, treatments, present medications (if any), known sensitivities/ allergies to medication or food and any other matter relating to your health.
  4. Follow the treating doctor and other healthcare provider’s instructions.
  5. Take medications as ordered.
  6. Give your written General Consent for assessment and treatment by the Physician and other licensed healthcare professionals when they visit you on your request for medical consultation.
  7. Give your written informed consent before an invasive procedure or any other treatments that entail requirements of written informed consent (according to DHA Rules and Regulations) after receiving all information that you may need.
    This must include the procedure benefits, available alternatives and all possible risks (if any).
  8. Hold the responsibility for refusing treatment or not following the treatment plan, after being informed of the consequences.

  10. upon request from your treating physician.
  11. Be available at your location (home/ hotel/office) on Doctor’s visit at the time of appointment. If you want to cancel or reschedule your appointment, promptly inform at least 1 hour in advance to the 24×7 Telephone or WhatsApp +971 52 967 7051
  12. Respect the priority given to elderly, children, and new acute illness (non-emergency) cases if you are already undergoing treatment with us and asking for a follow-up appointment. Appointments may be rescheduled or postponed at the last minute due to unavoidable circumstances
  13. Observe safety regulations, including the ‘no smoking’ policy, maintaining the cleanliness of your place, hand hygiene, etc
  14. Ensure that financial obligations due for Wellness Doctor Home Health Care, if any, are fulfilled promptly.
  15. Give the requested samples and take the medical checkups on time, without delay.
  16. Keep your valuables safe under your custody. If you have some valuables with you and if it results in the inconvenience of the clinical care provided, then it should be removed and kept aside under your custody
